Our district has done this with 4th graders for several years. We've customized a standard questionnaire to absolutely minimize the chance that 10 year olds will get led into dark places, and every year, some do anyway.
Do you ever think about hurting yourself? "Sure, I hate it when I hurt myself."

I don't think a single child has ever been helped by this procedure in my experience but a number of kids and families have been freaked out-- and that's in a setting where most of the staff is highly dubious about it, not a bunch of socialworktruebelievers.
Abigail Shrier is mentioned but not her book which directly addresses this subject of how "helping" can hurt: its called "Bad Therapy" and its a gem. I recommended it to a counselor and she later called me back: "ohmygod, I think I'm a bad therapist!"
